Background/Education Required:. Bachelor's Degree in Human Services or in a management related course of study. Master's Degree preferred. Five years' experience in human services or related field. Initiate and maintain contact with local OPWDD Division of Quality Improvement (DQI), Mental Hygiene Legal Services (MHLS) and The NY State Justice Center regarding incidents/investigations.. Provide, implement, and monitor recommendations as a result of investigations and audits and ensure follow-through of internal/ external auditing, as assigned.. Audit/review department, program and agency business practices and provides recommendations.. Support other agency departments in understanding and following regulations and best practices.. Assist with operation of the Quality Committee.. Supervise and occasionally conduct investigations of allegations, incidents and injuries according to OPWDD guidelines. Conduct interviews, examine documents, collect evidence, and write reports concerning allegations of abuse and/or other incidents requiring an investigation.. Track and record all details of incident/investigation processes in required software tracking program.. Complete any needed follow up tasks relating to investigations.. Occasionally present investigation information at routine Incident Review Meetings.. Maintain the highest level of confidentiality throughout the investigative process.. Attend OPWDD investigator/incident management trainings.. Perform other duties as assigned or directed.. Conduct training for newly hired staff.. Complete quarterly and annual trend reports..
